Insurance Coverage

For your first visit to our office, please be prepared to bring the following information with you.

• Your complete demographic information, including address and phone number
• Insurance Cards
• Emergency Contact Information

Please know that with each visit you will be asked to verify this information, if you have any changes please let us know.

Before you are seen by one of our physicians, you should know what your policy coverage is. How much your co-pay, deductible or service benefits are, such as immunization and well visits. We are happy to file your claims to the insurance company. Please also realize many insurances do not cover full amounts, and you may owe for your service. We ask that you are prepared to pay your balance at the time the service is provided. If needed, we will work out payment plans provided you make a payment to start with.
