Linus Minnick, MD, a local stalwart for the Churubusco community built the Churubusco Clinic more than 50 years ago. Also a farmer, Dr. Minnick wanted to ensure residents in Churubusco and the surrounding communities had access to healthcare close to home. After closing for a few months in 2003, Thomas Kintanar, MD, reopened the clinic under the name Churubusco Family Medicine in August 2004. He oversaw operations until June 2009, when he sold the practice to St. Joseph Medical Group.

Staff Bios
Jeffrey L. Witt, MD
Family Medicine
A Fort Wayne native and Snider High School graduate, Jeffrey L. Witt, MD, joined St. Joseph Medical Group in August 2010 after serving as the associate medical director of the Fort Wayne Medical Education Program for seven years.
Dr. Witt graduated summa cum laude with a bachelor's degree from Ball State University in 1981. He earned a medical degree from the Indiana University School of Medicine in 1985 and completed a clinically oriented residency in family practice with a focus on emergency medicine and obstetrics at FWMEP in 1988.
From 1981 to 1992, Dr. Witt served in the United States Naval Reserve. He served as the director of outpatient surgery at Marine Corps Headquarters in Quantico, Va., from 1988 to 1989, and he served as senior medical director of a branch clinic in Sasebo, Japan, from 1989 to 1992.
From 1992 to 1995, Dr. Witt was in private practice in Richmond, Ind., where he also served as medical director at Cherish Nursing Home and Cardinal Home Health Care and worked as a medical advisor to the Ivy Tech medical assistant program.
In 1995, he moved to Huntington, Ind., and joined Family Practice Associates, where he worked eight years. During this time, he became an associate faculty member for the Indiana University School of Medicine and a clinical faculty member at FWMEP. He joined FWMEP full time in 2003.
Dr. Witt is a member of the American Academy of Family Physicians, the Indiana Academy of Family Physicians and the American Medical Association.
Lisa Foldesi, NP
Nurse Practitioner
Nurse practitioner Lisa Foldesi has been working collaboratively with Dr. Kintanar and taking care of patients at Churubusco Family Medicine since December 2005. Before coming to the clinic, she worked with Dr. Kintanar at Associated Family Medical Consultants in Fort Wayne. Lisa sees patients at the clinic daily.
Before becoming a nurse practitioner, Lisa was the supervisor of the diagnostic stress testing department at Fort Wayne Cardiology, and she worked as a registered nurse in the cardiac catheterization labs at Lutheran Hospital in Fort Wayne and Wood Johnson University in New Brunswick, N.J. Early in her career, she was a registered nurse in the medical/surgical and progressive cardiac care unit at St. Joseph Medical Center in South Bend.
Lisa earned a diploma in nursing from Memorial School of Nursing in South Bend in 1988. She earned a bachelor's in nursing in 2000 and a master's in primary care nursing in 2005 from Indiana Wesleyan University, Fort Wayne.
Naveen Lal, MD
Cardiology

Dr. Lal is board certified in internal medicine, cardiology, interventional cardiology, nuclear cardiology and echocardiography. He sees patients at the clinic every other Monday from 1:30 to 5 p.m.
Dr. Lal earned a medical degree from Maulana Azad Medical College, University of Delhi, New Delhi, India, in 1988. He completed a residency in internal medicine at the Post Graduate Institute of Medical Education and Research in Chandigarh, India, where he also served as a faculty member in the Department of Internal Medicine. He later completed an internship in internal medicine at Prince George's Hospital Center in Cheverly, Md., in 1993, and a residency in internal medicine at Baylor College of Medicine in Houston in 1994.
Dr. Lal completed a fellowship in cardiology research at the National Heart Lung and Blood Institute, National Institute of Health in Bethseda, Md., in 1996. He also completed a cardiology fellowship and an interventional cardiology fellowship at the world-renowned Texas Heart Institute, Baylor College of Medicine in Houston in 1998 and 1999, respectively.
Dr. Lal, who is a member of Allen County Cardiology, has been practicing in Fort Wayne since 1999, specializing in cardiovascular disease.
Thomas Kintanar, MD
Family Medicine
Thomas Kintanar, a member of Associated Family Medical Consultants, has been treating patients at Churubusco Family Medicine since August 2004. He sees patients one day a month.
Dr. Kintanar, who is a member of Associated Family Medical Consultants, earned a bachelor's in microbiology from Indiana University in 1978. He earned a medical degree from Southwestern University, Matias H. Azner Memorial College of Medicine in the Philippines in 1983, and he completed a residency in family medicine at the Fort Wayne Medical Education Program Family Residency program in 1986.
Dr. Kintanar serves on the board of trustees at St. Joseph Hospital and is the hospital's director of medical education. He also serves on the board for the Fort Wayne Medical Education Program and is medical director of Glenbrook Nursing Home, Wood Youth Center, Heatland Hospice, Lutheran Homes and Coventry Meadows.
Dr. Kintanar is board-certified by the American Board of Family Practice.
