Theresa Cabe, M.D. is a graduate of the University of Santo Tomas Medical School, Philippines. She completed her residency at the Fort Wayne Medical Education Program. Following her residency training, Dr. Cabe practiced with Brooklyn Medical Associates from 1996 through 1999 at BMA's Monroeville office with Don Giant, M.D. From 1999 through September 2006, Theresa worked at StatCare and RediMed. Her hours worked at these urgent care centers afforded her the time to start a family and have some home time with them. Dr. Cabe and her husband, Emmanuel Cabe, M.D. (also a Brooklyn Medical Associates physician at the West Jefferson office) are the parents of two daughters.

Dr. Cabe is board certified in family medicine and is a member of the American Academy of Family Physicians, Indiana State Medical Association, Indiana Academy of Family Physicians and the Fort Wayne Medical Society.

A member of St. Joseph Medical Group Inc., Ayodeji O. Rosanwo, MD, is a family practice and geriatrics board-certified physician who practices at Senior Health Services. Dr. Rosanwo specializes in geriatric concerns such as diabetes, Alzheimer's, nursing home depression, cardiovascular events and influenza surveillance. Dr. Rosanwo earned a medical degree at the University of Lagos, Nigeria, in 1985, and completed his post-graduate work at Metro Health Medical Center in Cleveland, Ohio.

Denise M. Smith, D.O., recently returned to St. Joseph Medical Group as a primary care physician after a one-year hiatus from the group. A native of Fort Wayne, Dr. Smith entered the medical field as a radiological technologist in 1978 and worked as a radiographer for several years before entering medical school. She graduated from the Kirksville College of Osteopathic Medicine in Kirksville, Mo., in 1993. Since completing a residency in family practice medicine at the Fort Wayne Medical Education program in 1996, she has worked in numerous capacities as a primary care and emergency medicine physician. Dr. Smith specializes in end-of-life issues for primary care patients.

Dr. Smith is a member of the American Medical Association, the Indiana Association of Family Physicians, the Indiana State Medical Association and the American Academy of Family Physicians.

A Fort Wayne native and Snider High School graduate, Jeffrey L. Witt, MD, joined St. Joseph Medical Group in August 2010 after serving as the associate medical director of the Fort Wayne Medical Education Program for seven years.

Dr. Witt graduated summa cum laude with a bachelor's degree from Ball State University in 1981. He earned a medical degree from the Indiana University School of Medicine in 1985 and completed a clinically oriented residency in family practice with a focus on emergency medicine and obstetrics at FWMEP in 1988.

From 1981 to 1992, Dr. Witt served in the United States Naval Reserve. He served as the director of outpatient surgery at Marine Corps Headquarters in Quantico, Va., from 1988 to 1989, and he served as senior medical director of a branch clinic in Sasebo, Japan, from 1989 to 1992.

From 1992 to 1995, Dr. Witt was in private practice in Richmond, Ind., where he also served as medical director at Cherish Nursing Home and Cardinal Home Health Care and worked as a medical advisor to the Ivy Tech medical assistant program.

In 1995, he moved to Huntington, Ind., and joined Family Practice Associates, where he worked eight years. During this time, he became an associate faculty member for the Indiana University School of Medicine and a clinical faculty member at FWMEP. He joined FWMEP full time in 2003.

Dr. Witt is a member of the American Academy of Family Physicians, the Indiana Academy of Family Physicians and the American Medical Association.
